Patient Rights and Responsibilities

Geriatric and Palliative Care Associates upholds the rights of every patient under Medicare and Medicaid regulations. We are committed to delivering care that respects each patient's dignity, privacy, and individual needs.
Patient Rights:

  • To receive high-quality, compassionate geriatric and palliative care that is free from discrimination.

  • To be informed about their diagnosis, treatment options, and care plan.

  • To participate in decisions regarding their care and treatment.

  • To receive services in a safe and respectful environment.

  • To privacy, confidentiality, and protection of their medical records.

  • To file a complaint or grievance without fear of retaliation.

  • To receive information about charges and services covered by Medicare, Medicaid, or insurance.

Patient Responsibilities:

  • To provide accurate information about their health and care preferences.

  • To participate actively in their care plan and communicate their needs.

  • To treat caregivers and staff with respect.

  • To inform the care team of any changes in their health condition.

  • To meet their financial obligations for services not covered by insurance.

Medicare Beneficiary Information

Geriatric and Palliative Care Associates provides care under the Medicare program, ensuring patients receive the necessary medical support for chronic and complex conditions.
Your Medicare Rights:

  • To receive care for your medical conditions under the Medicare program.

  • To receive services including nursing care, pain management, and emotional support.

  • To choose your healthcare provider.

  • To continue receiving care as long as you are eligible.

  • To change your care plan or provider at any time.
